1. Contact for Children at school.
A print out of the emergency contact showing Francisco clearly stated as the step-parent and the second contact with emergencies with our children. Previous FEEPAY with the schools stated Francisco as a contact and parent. This shows continuation.
2. Lease.
I bought the mobile home we are living in back in 2008. The mobile home doesn’t have a mortgage as it was already paid for. We rent land that is a month to month lot payment. Francisco is on the month to month lot payment responsibilities.
3. Bank Statements.
Bank statements for three years from 2015 to current. Showing a well-established shared account of day to day living and household expenses. In addition to the letter from the Bank stating Francisco was added to the account July of 2015.
4. Health Insurance verification showing all under one policy as a family
5. Life Insurance policies.
- Previously submitted showing Francisco as a beneficiary sending again as requested.
- Previously submitted showing Renee as a beneficiary. Started new employment in July 2018 and sending documentation of new life insurance policy with new employer and Renee as beneficiary.
6. Insurance car policy showing continued insurance in both of our names and residence.
7. Joint Sam’s Club Membership

9. Joint Utility bills showing shared responsibility for household expenses along with a 2 year statement showing history of joint responsibility.
10. Joint credit card in both of our names. (Smile Generation and Target)
11. Car loan in both of our names showing joint responsibility and car title showing joint ownership and residence

15. Copy of airline reservation for family vacation in August 2018
16. Affidavits from the following people U.S. citizen family and friends who have known Francisco and I throughout our marriage and before our marriage.
17. Pictures showing continued relationship with regards to ourselves, family, friends and vacations.
18. Death planning. Will or power of attorneyI actually sent a lot of the same stuff they lost half of it when it was moved from CA to VT. It is very ridiculous that I have seen people that originally sent far less get approved. My husband and I have been together 8 years at this point. Dated for 4 and married for 4.

• Letter from our bank showing he has been a joint account holder since July 2015.
• Statement from our bank showing both as account holders.

• A copy of our Tax return from 2014 and 2015
• Copies of car insurance notices showing both of our names
• Copy of his life insurance policy showing beneficiaries
• Copy of my life insurance policy showing beneficiaries
• Copy of our health insurance showing joint policy on eye and dental.
• Copy of his health insurance through his own employment
• Utilities bills showing both of our names and same homestead
• A copy of our airline reservations for a vacation that we took as a Family in August 2016 visiting his family in Mexico.
• Transcript to school showing address on file.
