Describe the claim
Name of the provider (with billing office number)
Is this member in collections?
Is this a pharmacy issue?
What is your expected resolution (i.e. you may want to put something like I expect anthem to pay more on this claim, etc., whatever it is that you are hoping to be the outcome to this situation)?
