To get your rebate just download and fill out the form below and mail it along with a copy of the receipt with qualifying purchase(s) circled, plus the original UPC code(s) from the package(s).
所以一封信+ receipt的复印件+ UPC codes

你仔细看看，有些Mail in 的UPC都是贴纸，撕下来就好

我买的那个锅只有塑料包装。。拆开之后就扔了。。怎么弄到UPC阿。。求答
