时间：8:30 am – 4:00pm
地点：Monrovia  
工资：$21up /小时 DOE (面试后议)

***必须资格***
1. 懂英文
2. 精通囯语（交谈，閲读，中英翻译）
3. 有美国硕士文凭 (或学士文凭加2年以上社工经验)

4. 文凭需要Psychology,Sociology, Social work, Gerontology, Healthcare, 相关专业.
5. 可以合法在美国工作

工作简介：

/Monitor andassess participant’s mental/physical/emotional status, provide care and supportaccordingly.  

/Completepsycho-social assessments for participants.
/Process all inquiries in a timely manner andmaintain tracking documentation.

/Meet withfamily members or referring agency, and determines appropriate program.

/Maintainprofessional attitude in situations; be caring to the aging / vulnerablepopulation.

/Provides fullcase coordination include precise English to Chinese translation and interpretationof governmental and health care programs and policies.

/Completequarterly assessment, progress notes, six-month reassessments, and initial assessment.

/Dischargeplanning for all participants.

/Develop monthly support group sessions.  

/Serve as amember of the multi-disciplinary team and participate in initial assessments,care planning, and on-going re-evaluation of participant's needs.

/Be sensitive and alert ofenvironmental safety and make appropriate adjustment.   
/Conduct in-home assessment as needed.
