急！！！EAD卡丢失，怎么补办？

急！！！EAD卡丢了，非USCIS问题，该怎么补办呢，什么流程，需要什么文件？要不要公司的offer什么的？因为是挂靠的公司，会不会有什么风险？

Employment Authorization Document (for students on OPT, or for J-2 dependents with work authorization): If you misplace your EAD card or it is stolen, you will need to file a new application (including the application fee) through USCIS. It may take up to 3 months to replace a lost or stolen EAD card. The following documents are required:
Form I-765 (Application for Employment Authorization)
$410 check or money order payable to "Department of Homeland Security"
Copy of I-20 signed within the past 30 days (contact GEC for a new signature)
Copies of biographical page of passport, US visa, and I-94
2 passport-size photos
Copies of any previously issued EAD cards
